We know requesting records can sometimes feel complicated, so we’ve created a simple process to help. Because your information is private and protected by law, we follow specific steps to make sure your records are handled safely and released without delay once everything is in place.

Requests from individuals who are not the client, or the parent/guardian of the client, require submission of a completed HIPAA Consent to Release Information Form before any information can be discussed or released.

Some requests – like letters of attendance, court or school forms, or other administrative documents – have a small fee. These fees don’t apply to your medical records.
