Most Major Insurance Plans Accepted – Verify Your Coverage Today!
Payment is required before services are rendered. We will accept cash and/or credit cards. Payment will include any unmet deductible, co-insurance, co-payment amount, or non-covered charges from your insurance company. If you do not carry insurance, or if your coverage is currently under a pre-existing condition clause, payment in full is expected at the time of you visit.
Insurance
We are participating providers with several insurance plans. We will file all of these insurance claims. A list of these insurance plans is available upon request. Please remember that insurance is a contract between the patient and the insurance company and ultimately the patient is responsible for payment in full.
Reserved Time-Missed Appointment Fee
Broken appointments represent a cost to us, to you and to other patients who could have been seen in the time set aside for you. Once an appointment is scheduled, you will be expected to pay $50.00 if it is not cancelled unless you provide a 24 business hour advance notice of cancellation. (For example, to cancel an appointment for 9AM on Monday, you would need to call before 9AM the previous Friday.) These charges cannot be billed to your insurance company. Please help us serve you better by keeping scheduled appointments and calling the office at least 24 business hours prior to your appointment time if you must cancel. Excessive cancelations of scheduled appointments may result in discharge from the practice.
Forms Fees
Completing insurance forms, copying medical records, writing letters etc. requires office staff time and time away from patient care we require pre-payment for completing forms, copying medical records, notarizing, or for extra written communication by the clinician. Form Completion $75.00 per document. Treatment Status Letters $10.00. Copying fees for Medical Records is .75 cents for each page. Alssaro Counseling Services, PLLC will have 15 business days in which to copy records before making them available for patient to pick up, and these 15 days will commence after payment for copying has been received and after patient has signed this form authorizing records’ release.
Court Action and Legal Fee
Even though you are responsible for the testimony fee, it does not mean that the testimony will be solely in the clients favor. Therapists can only testify to the facts of the case and, if qualified to do so by the court.
If the therapist is to receive a subpoena, then the attorney or office staff will need to call the office and set up a time for the subpoena to be served during office hours. We request a minimum of 72 business hours notice of any Court appearance so that schedule changes for therapist clients can be made within a reasonable time frame.
When it comes to court action, the following fees are in effect:
- 1. Preparation Time (including submission of records): $400.00
- 2. Phone calls: Flat Fee $90.00 per call
- 3. Depositions: $250.00/hour
- 4. Time required in Giving Testimony: $250/hour
- 5. The minimum charge for a court appearance: $1500
A retainer of $1500 is due at least 5 business days before the scheduled court appearance. The remainder of the costs will be billed after the court appearance and will be due upon receipt. If the therapist is subpoenaed and the case is reset with less than 3 business days notice prior to the beginning of the day of the scheduled subpoena, trial, and/or testimony is not given, then the client will be charged $500 (in addition to the original retainer of $1500 for having to appear in court).
